AGMA Health Plan A & A1 Notice - At-Home COVID Tests: On January 10, the Biden Administration issued guidance providing that group health plans would be required to cover the cost of FDA-approved over-the-counter (OTC) at-home COVID-19 tests purchased on or after January 15, 2022, subject to a limit of 8 per month for each covered individual. These tests are to be provided without participant cost-sharing, preauthorization, or medical management, during the public health emergency for tests that are intended for diagnosis or treatment (but not to include screenings for employment, school or recreational purposes).

Since this requirement was announced and made effective with little lead time, the AGMA Health Fund and Aetna are working together on a program to provide this benefit through the Aetna pharmacy program. As soon as Aetna provides us with additional information, we will share the details on our website.

2021 TAX FORMS ARE AVAILABLE For Plan A participants in 2021, Form 1095-B was mailed out. If you haven't received it, please contact our office and we will mail or email it to you. For retirement participants who were paid out in 2021, Prudential issued Form 1099-R for distribution after September 1, 2021, and the AGMA Fund Office issued Form 1099-R for distribution prior to September 1, 2021.

Retirement Plan Changes:
Member balances with AGMA Retirement are now managed by Prudential Retirement starting September 1st, 2021. This change will help us provide our plan members with improved retirement planning resources and benefits - choosing your own investment mix, daily valuations, online account management, beneficiary updates, and 24/7 access to your account online and via an automated phone system.

For online information please visit the Prudential Account Login website. This resource will allow you to allocate your investments, elect beneficiaries, request withdraw/rollover requests, along with many other tools that are helpful when making these important decisions for your future. If you are living overseas and want to set up your Prudential account online, you will first need to call the Prudential Call Center toll free number at 1-877-PRU-2100 (001-877-778-2100) which works internationally. The Prudential Call Center is open Monday-Friday 8am-9pm ET.

Effective April 1, 2022, Empower officially acquired the full-service retirement business of Prudential. As you navigate the website, you will notice references to Prudential and Empower as they transition the experience for all customers. You should have received additional communications following the closing date and you should start to see the Empower logo when you access your account online or in the mobile app, if you have downloaded it. Online Portal: The AGMA Fund Office member portal is now online and available at This portal is set up to allow members to view employer contributions and eligibility directly. Initially, the portal shows contributions to the AGMA Retirement Plan; additional updates will allow members to see contributions and eligibility for the AGMA Health Fund.

Members who have provided their electronic consent should have received a welcome email. If you are experiencing a problem with the link in that email, please simply enter the portal address ( and then provide the temporary user ID and login from the email. To sign up for the portal and to provide your electronic consent to have materials emailed to you, please complete and return the electronic consent form to and we will have the system generate your new login information. If you wish to only access the portal, please contact the Fund Office to be set up.

Email Security: When we communicate back to you, your email may come through as a link to ZixMail. This is the service that we use to communicate securely with our members in order to protect your Personal Health Information (PHI) and other personally identifiable information. Once you have set up an account for ZixMail, please use this service to respond to our emails securely. Please note that secure emails will expire after 30 days to protect your information.

The Plans are administered by a Board of Trustees made up of an equal number of Union and Employer representatives.
